Bulk Warranty Claim Submission

Use this page to submit multiple warranty claims at once by uploading a CSV file. Start by downloading the template below, fill in the required claim details for each row, and then upload the completed file. The system will validate and process each claim individually, providing you with a detailed results summary when finished.

Field Reference

Column	Required	Notes
`serial_number`	Always	Unit serial number
`model_number`	Always	Unit model number
`failure_date`	Always	Format: `yyyy-MM-dd`. Cannot be in the future.
`failure_codes`	Always	Comma-separated failure codes. At least one required. Each row will likely have different failure codes. Use the Failure Code Lookup below to find GUIDs for each claim.
`flowcenter_serial`	Conditional	Required when any selected failure code is a Flow Center failure
`flowcenter_model`	Conditional	Required when any selected failure code is a Flow Center failure
`hss_serial`	Conditional	Required for certain HSS-related failure codes. See the Failure Code Lookup below for which codes require it.
`hss_model`	Conditional	Required for certain HSS-related failure codes. See the Failure Code Lookup below for which codes require it.
`leak_type`	Conditional	Required for certain leak-related failure codes. Use `A - Leak is to atmosphere` or `B - Water to refrigerant mixing`. See the Failure Code Lookup below for which codes require it.
`technical_contact`	Conditional	Enter the technical contact's name (the technician familiar with the unit in question). Required for certain failure codes. See the Failure Code Lookup below for which codes require it.
`technical_contact_phone`	Conditional	Required under the same conditions as `technical_contact`
`po_job_name`	Conditional	The PO / Job Name to be associated with this warranty transaction
`need_parts_shipped`	Always	`true` or `false`
`parts_purchased`	Always	`true` or `false`. This is generally false when `need_parts_shipped` is true.
`reimbursement_details`	Conditional	Required when `parts_purchased` is `true`. The PO / Job Name / Sales Order # associated with the purchased parts transaction.

`homeowner_last_name`	Optional
`homeowner_street`	Optional
`homeowner_city`	Optional
`homeowner_state`	Optional
